Jak of All Trades Fest is back! An action-packed, family-friendly, one-day, private event held at Jakprints showcasing all-things Jak! RSVP is required to attend. Anyone under the age of 18 must be accompanied by a RSVP'd adult.

LIABILITY & PUBLICITY RELEASE

By submitting this RSVP, I am confirming my participation in the Jak Of All Trades Fest (“Event”) taking place at Jakprints located in Cleveland, Ohio on September 3, 2011.

With this RSVP I agree to the following:

  1. I acknowledge that when attending the Jak Of All Trades Fest I may be directly or indirectly involved in potentially hazardous activities.
  2. I agree to accept full responsibility for any and all financial obligations incurred as a result of any medical assistance and related expenses provided in connection with any injuries arising out of my direct or indirect participation in the Event.
  3. I agree that Jakprints, their subsidiaries, affiliated companies, participating venues, sponsors, property owners, community partners, and all of their respective officers, directors, employees, representatives and agents, and all other persons or entities associated with this Event, will have no liability whatsoever for and shall be forever held harmless by me against any liability for any injuries, losses or damages of any kind to persons, including death, personal or property damages or losses resulting in whole or in part, directly or indirectly, from my participation in the Event.
  4. I understand that there are risks associated with consumption of alcoholic beverages and agree to fully abide by the laws of the State of Ohio.
  5. Additionally, I give my consent to Jakprints and their participating event sponsors and those acting under their authority to use my name, photograph, voice and/or likeness, without compensation, unless prohibited by law, in any and all advertising and promotional materials, in all media now known or hereafter discovered, without notice, review or approval.
  6. I warrant and represent that I have read this Release and fully understand the contents, effect and legal impact. I am eighteen (18) years of age or older and that I have the right to enter into contracts in my own name. I understand that any minors accompanying me to the Event must remain under my supervision at all times. I (along with their parents/legal guardians) agree to hold the above mentioned parties harmless of any injuries, losses or damages of any kind, including death, personal or property damages or losses resulting in whole or in part, directly or indirectly, from minors participation in the Event.
